Comments: The Discussion establishes the various anomalies associated with dyskeratosis congenita. The patient studied in this case did not show marrow abnormalities which suggests that this particular case is an incomplete expression phenotype of this condition. The diagnosis, prognosis, effect of age on the disease and possible treatments have been elaborated in the discussion section. Several relevant literatures have been cited to support the discussion. Due to the unavailability of intraoperative frozen tissue analysis equipments in the operation theatres at the hospital the surgeons used limited surgical approaches such as to palliative surgical removal of the lesion without resection of deep anatomical structures and unilateral neck dissection to treat the patient. The study concludes that no postoperative complications were observed post one month of the surgery.

Comments: This study presents the case of a 64-year-old Libyan male suffering from dyskeratosis congenita.  Dyskeratosis congenita is an inherited disease which usually affects multiple systems of the body. The patient studied in this report has a number of medical complications and the fact that he had to be treated during the COVID-19 pandemic makes the case all the more difficult. The case report discusses several important issues that should be considered while diagnosing dyskeratosis congenita. Surgical removal of the lesion and postoperative chemoradiation was used to treat the malignant neoplasm in the buccal mucosa of the patient. No clinical sign of relapse or new lesion was observed even a month after the surgery. No postoperative unusual clinical or abnormal laboratory findings have been observed in the patient.
