The Discussion explains the challenges in treating the patient in the present case clearly. It also offers useful suggestions with respect to the treatment regimen. Due to the ineffectiveness of antibiotic therapy along with drainage in the present case, the decision was made to perform nephrectomy, after which the patient could not make it. The study is well supported with figures. Based on occurrence of death despite standard management, the study concludes that general condition, age, and concomitant diseases have a significant effect on the outcome of treatment.

Comments: This case report presents the case of a 62-year-old woman with multiple comorbidities, with a diagnosis of Emphysematous pyelonephritis (EPN) whose condition did not improve despite the standard treatment approach. Emphysematous pyelonephritis is a type of necrotizing infection that results in the production of gas in the renal parenchyma, renal collecting duct, or perinephric tissue. This study holds significance as studies in the last two decades show a very high mortality rate of 21% for this condition.
