Comments: This report presents a 49-year-old patient with a brown linear hyperpigmentation on the left thumb nail without involvement of the nail edges following no symptoms of internal and genetic disease or dermatoses of the nail bed. This manuscript provides a brief account on the causes of appearance of the longitudinal melanonychia along with the clinical signs and symptoms of its occurrence and describe its treatment with picosecond laser. It also states that since patients with melanonychia are often misdiagnosed, differential diagnosis is necessary for the entire nail.
