Comments: The apt analysis of this study leads to a conclusion that highlights the underlying pathophysiologic mechanisms associated with high output cardiac failure. It states that obesity, arteriovenous fistula, liver disease, hyperthyroidism and severe anemia result in high output heart failure which is characterized by increased metabolic demand and decreased systemic vascular resistance. Further research is required to better understand the regulation of vascular tone and fluid retention in obese patients as obesity is one of the most potent risk factors for this disease.

Comments: This review article provides a valuable insight into the underlying pathophysiologic mechanisms, prevalence and clinical causes of high output heart failure. High output heart failure is associated with low systemic vascular resistance, reduced arterial-venous oxygen gradient and an elevated cardiac output and it occurs when elevation in overall cardiac output fails to meet the metabolic demands of the body. Obesity, arteriovenous fistula, liver disease, hyperthyroidism, severe anemia and mitochondrial disease contribute to the pathogenesis of this disease. This study is of significance as the underlying etiology predicts the clinical manifestations of high output cardiac failure.
