Oral Methadone for Cancer Pain Management at the End of Life: A Forgotten Drug

™      Significance of the Study: This study describes a single center 9-year experience of opioid rotation to methadone, the population profile, efficacy of methadone in cancer patients of the palliative care service. Rotation became necessary, reducing pain and manageable adverse events, without major changes of the initial and final dose. Further, controlled studies are required to establish the role of oral methadone rotation and its benefits in patients with difficult pain control and other characteristics such as type of cancer, type of pain, history of addiction.

 

™      Fit with Journal: The manuscript is highly suited for the journal Drug and Drug Abuse.

 

™      The performed experiments are quite convincing, and the observations are well discussed in the Discussion section. The figures and tables adequately justify the report findings. The conclusion is consistent with the evidences presented in the article.

 

 

™      The manuscript is mostly well-written except a few minor errors (already corrected in the galley proof), which are listed below –

 

§  In the 2nd sentence of 2nd paragraph of Introduction, “has” should be replaced with “have” and in the 3rd sentence of the same paragraph, “institution” should be replaced with “institutions”.

§  In the 3rd sentence of 2nd paragraph of Results, “of” should be inserted after “because” and the construction of the last sentence of that same paragraph is improper.

§  In the 4th sentence of the 5th paragraph of Results, “Interesting enough” should be replaced with “Interestingly”, “increased” should be replaced with “increase” and the framing of the last part of the sentence should be corrected.

§  In the 3rd sentence of the 4th paragraph of Discussion, “are” should be removed and in the 2nd sentence of the 5th paragraph of Discussion, “similar than our report” should be replaced with “similar to our report”.

§  Punctuations and articles, e.g., “the” were missing in several places throughout the text.
