Direct Oral Anticoagulants for Treatment of Venous Thromboembolism Associated with Cancer: A Systematic Review and Meta-Analysis

Purpose and Significance of Study: This systematic review and meta-analysis attempted to compare the efficacy and safety of direct oral anticoagulants (DOACs) with the well-established low-molecular weight heparin (LMWH) in patients with malignancy and venous thromboembolism (VTE), and then compare the safety and efficacy of the different DOACs. DOACs were found to be an acceptable alternative to LMWHs for the treatment of VTE in patients with malignancy, while there was no benefit of any one specific DOAC over another. This might aid physicians in the challenging decision-making about the choice of anticoagulation therapy in such cases. However, the study has a few limitations.

 

Fit with Scope of Journal: The manuscript is of very high interest for the journal Clinical Oncology and Research.

 

The results are clearly explained and adequately supported by data. The discussion explained the results in detail with respect to the four trials considered in the meta-analysis and illuminated the implications of the findings.  

 

o   The manuscript contains some minor errors (already corrected in the galley proof, except the highlighted ones, which require the author’s attention) listed below –

 

·       In the 3rd sentence of the 1st paragraph of Data synthesis and analysis under Methods, “occurred in” must be replaced with “bleeding occurring at”, and “contributed” must be replaced with “contributing”.

·       In the 3rd sentence of the 3rd paragraph of the Discussion, “bleeding” must be inserted after “major”.

·       In the 1st sentence of the 4th paragraph of the Discussion, “other” after “another” must be removed.
