Comments:
The Discussion encompasses the relevant findings from the previous studies on bleeding oesophageal varices and provides a thorough analysis of the findings and their implications. It explains the challenges in treating the patient in the present case clearly. It offers useful suggestions with respect to the treatment regimen. The Discussion section also adequately discusses the literature. The report is apt in concluding that oesophageal transection is a proven option for patients where endoscopic measures and balloon tamponade have failed and should remain in the armamentarium of the General Surgeon.

Further comments on the paper
Comments: This case report presents the case of a 32-year-old male patient who underwent oesophageal transection as a life-saving procedure with satisfactory outcome, together with a brief literature review on this topic. Acute bleeding from oesophageal varices presents in 70% of patients with liver cirrhosis. This study holds significance as despite significant advances in treatment, bleeding oesophageal varices remain a challenging condition. Surgical oesophageal transection for uncontrolled variceal bleeding is a last resort measure that is rarely needed due to modern endoscopic advancements and the introduction of endoscopic therapy has significantly decreased mortality from variceal bleeding.
