The case description is adequate. The Introduction lays down the basics for taking up the present study. The Discussion explains the challenges in treating the patient in the present case clearly. It offers useful suggestions with respect to the treatment regimen. It also adequately discusses the literature. The report is well supported with briefly explained figures. Methadone is considered a safe drug but has many side effects in high-doses. The Conclusion section is apt in recommending the necessity of a multimodal therapy including cardiovascular resuscitation (CPR), multiple thoracic drains insertion, high doses of vasopressors and haemodynamic monitoring for guiding therapy.

Comments: This case report describes the experience of a multimodal therapeutic approach in a 56-year-old patient with methadone-induced dilated cardiomyopathy who developed recurrent bilateral tension pneumothorax. This report is of importance as multiple recurrent tension pneumothoraces with insertion of 7 thoracic drains, consecutive to cardiovascular resuscitation (CPR) and mechanical ventilation, is quite rare in the existing literature.
