Comments: There are few issue found in the manuscript, which are as follows:

·       In 1st paragraph, 5th sentence is rewritten as “The group pointed conditions related to this condition as the basis of the recommendations”.

·       In 3rd paragraph, 4th sentence, “but it is usually studied separately because its biological behavior is more severe” is reframed as “but it is usually studied separately due to its more severe biological behaviour”.

·       Under Myocutaneous flaps, 6th paragraph, 2nd sentence, “although its necessary more studies … necrosis rate by author” is reframed as “although more studies are necessary to increase the global casuistry; all the studies were retrospectively carried out and a great variability was observed in relation to the necrosis rate by author”.

Comments: The manuscript provides various surgical aspects on locally advanced breast carcinoma (LABC) which is proposed by Brazilian Society of Oncological Surgery. It comes up with a review on different surgical techniques and complications associated with thoracoabdominal or myocutaneous flaps. Further, it provides relevant literature on the clinical aspects, the tumor conditions, response to neoadjuvant therapy, choice of flaps and tumor biology. The article also illustrates in detail the resectability, patchwork conditions in LABC patients. The author provides relevant recommendations on the diagnosis, surgical approaches and treatment associated with LABC. Lastly, it puts forth the different perspectives related to the understanding and management of the disease and its complications through various surgical techniques.
