Diffuse Midline Glioma, H3K27M-mutant Subtype, Confused for Viral Encephalitis: A Case Report

Review Data

Q: Is the topic relevant to the journal area of interest? Is it contemporary and interesting for

researchers?

A: Excellent

Abstract & Keywords

Q: Are all required components included in the abstract? Are the keywords appropriately chosen?

A: Very good

Goal

Q: Is the goal explicitly stated in the Introduction? Is its formulation clear and unambiguous?

A: Very good

Structure

Q: Is the paper's structure coherent? Is it in coherence with the goal of the paper?

A: Very good

Tools and Methods

Q: Are methods the author uses adequate and well used?

A: Very good

Discussion & Conclusion

Q: Is it related to the results presented before? Do you consider them as coherent?

A: Very good

Comments: The Discussion offers an adequately detailed analysis of the present case and probable reasons for the unfortunate outcome. It illustrates the criticality of early and correct diagnosis in similar cases.

Literature

Q: Does the author utilize relevant literature?

A: Good

Author's knowledge

Q: What is the level of the author’s knowledge? Does the author utilize all recent contributions relevant to the topic?

A: Good

Length

Q: Is the length of the paper adequate to the significance of the topic? Do you suggest shortening the paper without losing its value?

A: Good

Figures & Tables

Q: Does the author use them suitably? Are legend and notations clear?

A: Good

·       Figure 3 was not mentioned in the text and was therefore inserted.

·       In the Figure 3 legend, “HE stained” must be replaced with “HE staining”. Also, A & B are mentioned in the Figures but not in the legend.

Writing style

Q: Is it clear and understandable?

A: Very good

Comments: A few very minor issues were noted:

·       In the 5th sentence of Laboratory examinations under Case presentation, “Mycobacterium tuberculosis” must be italicized as it is a scientific name.

·       In the 2nd sentence of Treatment under Case presentation, “?” after “encephalitis” must be removed, and “peniclovir” must be replaced with “penciclovir”.

·       In the 2nd sentence of Conclusion, “considered” must be replaced with “consideration”.

·       Commas and articles (a, an, or the) were missing in a few places.

Further comments on the paper

Comments: This is a report of a case of H3K27M-mutant diffuse midline glioma, initially misdiagnosed as viral encephalitis. It is critical as this rare but highly malignant and aggressive intracranially occupying lesion is often confused with viral encephalitis while early diagnosis is crucial, and it is necessary to strengthen the mutual differentiation between diffuse midline glioma and other midline occupying diseases (like intracranial germ cell tumours, primarily in the third ventricle, supra saddles, and pineal region) or encephalitis. This case demonstrates the importance of differential diagnosis between viral encephalitis and diffuse glioma in patient outcome especially when the effect of anti-infection therapy is poor.

Q: Would you recommend this manuscript for further publication?

If you have any questions and clarifications you can write to the journal.
