Comments: This study is of significance as it is the first study that evaluates the feasibility and sustainability of a telemedicine intervention project of cardiovascular pre-participation screening (PPS) in 506 athletes, conducted by a non-profit organization. The aim of this study is to appraise the economic viability and the social contribution of telecardiology intervention in order to support an ECG based pre-participation screening in athletes as well as to evaluate the cost and benefits of this project to provide information about the sustainability of this programme. This study further recommends that PPS based on electrocardiogram (ECG) combined with telemedicine project can be used as an alternative diagnostic tool to diagnose sudden cardiac death (SCD) and sudden cardiac arrest (SCA) early in athletes. The major limitation of this study is that embedded costs differ from context to context rendering the generalizability of the findings disputable.
