This case series confirms the utility of flow cytometry in the initial diagnostic work up and post-therapeutic monitoring of Nodular Lymphocyte-Predominant Hodgkin Lymphoma (NLPHL). The Discussion offers a thorough and detailed analysis of the findings of the present study taking all the relevant literature into account. The study strongly recommends considering NLPHL in the differential diagnosis when such flow cytometry features are detected since reactive T-cells may have unique immunophenotypic features suggestive of NLPHL. It has the potential to help pathologists to improve diagnosis of NLPHL when evaluating the specimens obtained via fine needle aspiration or core biopsy.

Comments: This study reports the findings of sequential flow cytometric studies of persistent or recurrent lymphadenopathy in patients with Nodular Lymphocyte-Predominant Hodgkin Lymphoma (NLPHL) focusing on the comparison between needle core and excisional biopsies. It also proposes a cost-effective diagnostic algorithm using flow cytometric features and cytological features of needle core biopsy to guide further diagnostic workup. The study is consistent in providing additional evidence validating previous reports in the literature. The study further finds that the T-cell flow cytometry signature is highly predictive of histopathological diagnosis, and it might help to improve the diagnosis when the specimen is a needle core biopsy or histopathology features are atypical.
