Purpose and Significance of Study: This is the report of a case of Pneumocystis jirovecii pneumonia in a young patient with lung adenocarcinoma and EGFR mutation treated with afatinib, where the differential diagnoses posed a unique challenge in the midst of the COVID-19 pandemic. This case exemplifies the plight of cancer patients who are at greater risk of contracting the disease as well as the challenges faced by the oncologists in such cases.

 

The details of the case are aptly described in the Case report. The Discussion elaborates on the rationale behind afitinib administration to the patient, the implications of Pneumocystis jirovecii pneumonia in this case and the critical need for an early and accurate diagnosis and a multidisciplinary approach.

 

o   The manuscript contains a few minor errors (already corrected in the galley proof) listed below –

 

·       Throughout the manuscript “Pneumocystis jirovecii” must be italicized as it is a scientific name, and “j” must be in lower case.

·       Some words are misspelled, e.g., “pneumonitis” in the 7th sentence of the Discussion and “Pneumocystis” in the 8th sentence of the Discussion.

·       In the 2nd sentence of the Abstract and the 2nd sentence of the Introduction, “to contract” must be replaced with “of contracting”.
