AB - Clear cell “Sugar” tumor of the lung (CCSTL) is an atypical and mostly benign peripheral primary
pulmonary tumor of Perivascular Epitheloid Cell Tumors (PEComas) family. We presented a case of
CCSTL of a 59-year-old woman with a well-defined, lobulated, solitary pulmonary nodule of soft tissue
density located in the right lower lobe which is persistently enhancing to 120 Hounsefield Unit (HU) in the
venous phase. The patient underwent a right lower lobectomy for both the curative and the diagnostic
purpose. The pathological examinations and immunohistochemical studies proved the case as a benign clear
cell tumor. With only a limited number of cases reported, clinicopathological aspects and radiological
characteristics of this tumor are not proposed systematically. In contrast enhancement CT imaging, the
persistently up sloping enhancement characteristic of nodule like in this case might be helpful to add in
separating CCSTL from other lung neoplasms.
