A cranial MRI plain and contrast revealed a right thalamo-mesencephalic mass with beginning hydrocephalus. The patient underwent endoscopic transventricular septostomy, biopsy of thalamic mass with ventriculoperitoneal shunt insertion under endoscopic guidance. The official histopathology results revealed a WHO Grade IV glioblastoma. The patient was subsequently treated with chemoradiation.}
